On January 5, 1959, condidential [sic] informant T-1 furnished information from a strictly confidential file to the effect that the subject's full name is Medgar Wiley Evers^[strikethrough]0[strikethrough]^. He was ^Index^ born July 2, 1925, at Decatur, Mississippi. The name of Jessie Evers^[strikethrough]0[strikethrough]^, Box 181, Decatur, Mississippi, was reflected in the file. This is believed to be the name of the subject's father[word circled with note ^mother^]. Medgar Evers, during World War II had Army Serial Number 34874245. He registered for selective service at Biloxi, Mississippi, on July 3, 1943. The file reflects Evers has a scar over the left side of his temple. He was recorded as 5' 5" tall, weight 150 pounds, with light brown skin. In 1943, he was working for the Roco Company, his employer being Edward Rocco, ^[strikethrough]0[strikethrough]^ Biloxi, Mississippi. In 1943, Evers gave the name of Mattie Wansley, ^[strikethrough]0[strikethrough]^ Post Office Box 188, Decatur, Mississippi, as the person who would always know his address. His selective Service number is 22552579.
Evers also registered in 1948 under the Selective Service Registration Act of 1948. At that time he also gave his address as Decatur, Mississippi.
